The story of these children who live in Salinas, an isolated village in the southwestern Dominican Republic, features in a new BBC science series “Countdown to Life – the extraordinary making of you”.

Scientists interviewed a “Guevedoce” male in a remote Dominican Republic village where one in 90 babies turn from female to male during puberty.

Some children with a rare genetic condition appear female at birth but later develop a penis and testes around the time puberty begins.

Referred to as “Guevedoces” or “penis at twelve”, this condition was first discovered by Dr Julianne Imperato, an endocrinologist at the Cornell University, during the 1970s.

The Y chromosome of a genetically male fetus will transform the sex gland into testicles. In males, DHT is responsible for the formation of the penis. If you’re female and don’t make dihydro-testosterone then your tubercle becomes a clitoris. It is not until puberty, when another huge surge of testosterone is produced, that the male reproductive organs emerge.

A little boy named Carla is now going through the same transformation, aged nine. Further cases have since been seen in the Sambian villages of Papua New Guinea, although the Sambians often shun the children, unlike the Dominicans, who celebrate the change. Because the disorder is so widespread and accepted, the Dominican Republic now believes that there are three sexual categories, male, female, and pseudohermaphrodite.

In addition to determining the cause of this genetic disorder, Imperato-McGinley’s also observed that Guevedoces commonly have smaller prostates, leading to Merck’s development of the Blockbuster Drug Finasteride, which is still used to treat enlarged prostates and male pattern baldness. Despite being brought up as girls for the majority of their lives, most young guevedoces identify strongly as heterosexuals and express a disdain for women’s lifestyles.

The 24-year-old said doctors didn’t originally know what sex he was but he always felt more like a boy, according to the BBC. “I never liked to dress as a girl…”

“They used to say I was a devil, nasty things, bad words and I had no choice but to fight them because they were crossing the line”, Johnny said.

“When they bought me girls’ toys I never bothered playing with them”.
