Doctor, Shut Up and Listen

HARRISBURG, Pa. — BETSY came to Dr. Martin for a second — or rather, a sixth — opinion. Over a year, she had seen five other physicians for a “rapid heartbeat” and “feeling stressed.” After extensive testing, she had finally been referred for psychological counseling for an anxiety disorder.
The careful history Dr. Martin took revealed that Betsy was taking an over-the-counter weight loss product that contained ephedrine. (I have changed their names for privacy’s sake.) When she stopped taking the remedy, her symptoms also stopped. Asked why she hadn’t mentioned this information before, she said she’d “never been asked.” Until then, her providers would sooner order tests than take the time to talk with her about the problem.
Betsy’s case was fortunate; poor communication often has much worse consequences. A review of reports by the Joint Commission, a nonprofit that provides accreditation to health care organizations, found that communication failure (rather than a provider’s lack of technical skill) was at the root of over 70 percent of serious adverse health outcomes in hospitals.
I had seen a case like this once where a male presented with rapid heart rate and nervousness and anxiety and all test were run and found to be normal.
When he came to me after seeing many doctors I noted that he had a very well shaped body with good muscles and I asked him if he worked out in a gym and he said yes.
Then I asked him if he used any fat burners and he said yes.
Fat Burners have high doses of caffeine to boost the metabolism and the side effects are an elevated heart rate and nervousness.
Also when I asked the patient if any doctor asked him if he took any supplements..he said that he told other doctors No becos he thought a fat burner was not anything like a medication or supplement! So it was not the fault of the doctors here.
You see Doc is talking apples..patient is thinking oranges!
But most doctors could have missed the Fat Burner medication cos not many read Fitness magazines and Sports Journals every month like I do!
Also I make it a standard practice to ask everyone if they take supplements of any kind..including herbal remedy.
Sometimes most women deny they take weight loss medication cos they feel shy to admit it feeling embarrassed that they could be viewed as fat and vain.
But there is a way of going about this and not to make the patient feel bad.
Instead of asking "are you taking any weight loss medication?'
Ask them "Are you taking any medication that helps you maintain a healthy body weight?"
Then you will hear Yes Yes!
There are many patients who LIE to doctors too and hide many facts.
So sometimes even when someone says NO..take it as a Yes and investigate.
I have seen many who swear they are virgins but actually are pregnant!
That way male patients are a bit more open..they do not hide as much as females.
These days many female drink collagen based drinks which has a sea food component and causes allergies at times.
So for everyone who walks in..I will ask them a list of questions if they are taking this or that or anything.
For those who still hide I tell them directly...Since you say so..this would be the treatment but if you are taking something and not telling me that this medication will no do any good and might be even harmful.
Then they say "Ok OK...I am taking this"
For some males..they never want to admitted they visited a sex worker when they display frank signs of an STD.
So till they admit..no treatment!
Then after 1 day they will admit!LOL
